Care management and coordination services are available to any patient who needs help. This may include working with members of the care team to develop a comprehensive care plan.
Members of the care team who are involved in this process may include the provider, a care management nurse, a community health worker and/or an integrated behavioral health specialist.
The overall goals of care management/care coordination are to optimize the patient’s ability to care for self and to identify and coordinate needed resources and support.
